BACKGROUND AND OBJECTIVES: Neoumbilicoplasty aims to reconstruct an aesthetically pleasing new umbilicus following agenesis, malignancy, anatomical distortion, or umbilicus loss. Despite the wide variety of surgical techniques described, literature is scarce when it comes to standardized categorization of these as well as the clear definition of patients' selections, specific indications, final outcomes, and possible complications. According to available literature, this work aims to evaluate different surgical approaches, and correlate them to specific surgical needs, to simplify the surgical choice and patient management. METHODS: A systematic review was performed in December 2020 in PubMed, Web of Science, and MedLine Ovid databases according to the PRISMA guidelines. RESULTS: A total of 41 studies and 588 patients were finally included. On the basis of the evidence of the literature collected, we divided the studies into four groups according to the neoumbilicoplasty techniques: single suture or purse-string suture, single flap, multiple flap, and skin graft. Patients' surgical comorbidities, neoumbilicoplasty indications, and aesthetic and surgical outcomes were investigated. Direct suture and single and multiple flap techniques assured overall, satisfactory cosmetic outcomes with a low rate of surgical complications. Whereas suture-only techniques were chosen mostly by general surgeons/urologists in laparoscopic surgery, the single flap was the preferred method to reconstruct the umbilicus in open abdominal surgery or combined abdominoplasty with herniorrhaphy. Multiple flap and skin grafts were adopted in abdominoplasty-related umbilicus reconstruction, although the latter option showed impactful aesthetic and surgical complications. CONCLUSIONS: Umbilicoplasty can assure generally pleasant aesthetic outcomes with relatively low complication rates. Indications for specific techniques correspond to different patient populations and surgical scenarios.

BACKGROUND: Complex fractures of the carpal scaphoid with poles fragmentation, edges comminution, bone loss and non-union of fractures previously treated by screw fixation remain challenging for hand surgeons. The authors present the indications, advantages and results of scaphoid plating, underlining the importance of correct plate positioning well shaped onto the bone. MATERIALS AND METHODS: The study includes 11 patients presenting acute fracture with distal pole fragmentation, acute fracture with comminution and non-union after prior failure of screw fixation. All patients were treated with volar locked plate fixation, adding a cortical bone graft in cases of non-union. RESULTS: Bone consolidation was achieved in all cases; excellent outcomes in fracture healing and relevant improvement in symptoms and functions were obtained in non-union group that are consistent with the literature. Only one patient underwent early further surgery (first row carpectomy) with poor results. CONCLUSION: The treatment of the selected scaphoid lesions with volar locked plate is a surely efficient technique. The plate can be easily adjusted to the shape of the scaphoid and can achieve an adequate correction of bone deformity and high degree of stability both in non-union and fractures.

The aims of this study were to evaluate the impact of the COVID-19 pandemic on emergency and elective hand surgery in four Italian regions that had either a high (Lombardy and Piemonte) or a low (Sicilia and Puglia) COVID-19 case load to discuss problems and to elaborate strategies to improve treatment pathways. A panel of hand surgeons from these different regions compared and discussed data from the centers they work in. The COVID-19 pandemic had an enormous impact on both elective and emergency surgery in Italy, not only in highly affected regions but also - and paradoxically even at a higher extent - in regions with a low COVID-19 case load. A durable and flexible redesign of hand surgery activities should be promoted, while changing and hopefully increasing human resources and enhancing administrative support. Telematics must also be implemented, especially for delivering rehabilitation therapy.

Upper limb burn treatment represents a major medical and surgical challenge. Enzymatic escharolysis is a rather new technique to treat thermal burns in an easy and rapid way, as an alternative to the standard of care. The aim of the study was to investigate and describe the efficacy of treatment of upper limb burns with NexoBrid® in a non-burn referral center. All patients suffering from upper limb burns and admitted within 36 hours to the Hand and Microsurgery Unit of the ASST Sette Laghi from December 2016 to June 2018 were enrolled in the study. A retrospective analysis was performed, evaluating time to wound healing, time of hospitalization, and scar aesthetic appearance with patient and observer scar assessment scale (POSAS) and disabilities of the arm, shoulder and hand score (DASH). A total of 18 patients with burns involving the upper limb from December 2016 to June 2018 were treated. The mean TBSA% involved was 3%; 4 out of 18 patients suffered circumferential burns. The mean POSAS score was 14; the mean DASH score at 6-month follow up was 21, while it reduced to 11 at the last follow up visit. Enzymatic escharolysis is a novel, rapid and selective treatment option that allows early physiotherapy with overall satisfying functional results. We believe that enzymatic escharolysis should be considered, in most cases, as the standard of care in the treatment of upper limb burn wounds in non-burn referral centers.


Le traitement des brûlures du membre supérieur (MS) est un défi médico- chirurgical majeur. Le débridement enzymatique est une technique relativement récente, facile et rapide, représentant une alternative au traitement classique. Le but de cette étude est d'évaluer l'utilisation du Nexobrid® dans le traitement, hors CTB, des brûlures du MS. Les 18 patients souffrant de brûlure du MS, hospitalisés entre décembre 2016 et juin 2018 dans le service de microchirurgie et de chirurgie de la main du Groupement Hospitalier de Territoire Sette Laghi dans les 36 h suivant l'accident ont été étudiés selon une étude rétrospective évaluant le délai de cicatrisation, la durée d'hospitalisation, l'aspect esthétique de la cicatrice (échelle POSAS), la fonction du MS (échelle DASH). La surface atteinte moyenne était de 3%, 4 patients avaient une atteinte circulaire. Le POSAS moyen était de 14, le DASH moyen à 6 mois de 21, s'abaissant à 11 à la dernière consultation de suivi. Le débridement enzymatique permet une rééducation plus précoce, avec des résultats fonctionnels satisfaisants. Nous pensons que cette technique est à privilégier dans le traitement hors CTB des brûlures du MS.

PURPOSE: The aim of the present meta-analysis was to update the literature on the outcomes and complications of ACL reconstruction in patients aged 40 years and older. It has been hypothesized that patients older than 40 years of age may have comparable clinical outcomes to those of younger patients. METHODS: A systematic review of articles from 1996 to 2018 was completed using Pubmed, Medline, Cochrane Reviews, and Google Scholar databases using the keyword terms "anterior cruciate ligament reconstruction" and "middle-aged OR elderly OR over 40 OR age factors." Functional and clinical outcomes (International Knee Documentation Committee, Lysholm and Tegner score and KT-1000 arthrometer), complication and graft failure rate were evaluated. RESULTS: Eleven articles met inclusion criteria. In total, 306 middle-aged patients and 566 younger patients were included in this study. The mean age of patients > 40 was 49 ± 7 (range 40-75) years with a mean follow-up of 25 ± 9 months (range 12-68). The mean age of younger patients was 26 ± 2.7 (range 15-39) years with a mean post-operative follow-up of 26.7 ± 11.5 months (range 3-64). The results were slightly higher (but no significantly different) towards the younger group in terms of objective IKDC (P = n.s.), Lysholm (P = n.s.) and Tegner (P = n.s.) scores and knee laxity assessment (P = n.s.). Complication rate (P = n.s.) and graft failure (P = n.s.) were low even in this cohort. CONCLUSIONS: The present meta-analysis shows that patients older than 40 years achieve comparable clinical outcomes to those of younger patients following primary ACL reconstruction. This evidence may push the surgeons toward a more aggressive approach in this specific cohort of patients. LEVEL OF EVIDENCE: III.

INTRODUCTION: The medial plantar artery (MPA) flap in its anterograde form is considered the gold standard for heel reconstruction. This flap can be also raised distally for the reconstruction of the weight-bearing forefoot. However, terminal branches of the MPA, together with their connection with distal and dorsal systems, can be variable. Our objective was to provide a comprehensive anatomic description that could match all technical possibilities in raising the distally based MPA flap. A systematic review of indication outcomes and complications of the distally based MPA flap is provided. METHODS: According to PRISMA criteria, we systematically reviewed previous literature using the MEDLINE database concerning the MPA flap from 1977 to November 2018 using the keywords « Medial plantar flap ¼ OR « Medial Plantar Artery ¼. Anatomic variations, techniques, indications, outcomes, and complications were analyzed. RESULTS: All different vascular pedicles that may be used for the vascularization of the MPA flap were classified. Apart from the flap with a proximal flow, there may be five anastomotic connections from the superficial MPA to the plantar arterial network. Four dorso-plantar links supply the plantar network thanks to dorsal vascularization. Literature analysis of outcomes showed how the retrograde MPA flap may be unreliable with 14% of venous congestion rate and 9.3% of average flap loss, for a total average flap complication of 18.6%. CONCLUSION: This review provides the ultimate, clear picture of the complex anastomosis of the forefoot, with direct referral to surgical flap raising techniques, guiding surgeons during challenging reconstructions.

In this study, microscopic, histochemical and ultrastructural observations in human skin are presented, 8 months after an injection of a novel PEGylated filler. Morphological features demonstrated an excellent integration of the filler with the connective tissue components and an effective interpenetration with the ground substance. The filler appears uniformly distributed inside the hypodermis. No segregation or encapsulation of cells and other structures was observed nor evidence of immunological adverse reaction. Furthermore, observed ultrastructural modifications of fibroblasts supports a stimulatory effect of molecular components production of the extracellular matrix, contributing to the cutaneous connective tissue renewal.

Neauvia Stimulate® is a biocompatible, injectable hyaluronic acid (HA) filler (26 mg/ml) PEG cross-linked with 1% of calcium hydroxyapatite (CaHA) for facial soft-tissue augmentation that provides volume to tissues, followed by a process of neocollagenesis for improving skin quality. The aim of the present study is to evaluate the possible modulation of collagen synthesis after treating human fibroblasts cultured in vitro with the product (Lot. 160517-26-1/2 PEG). The experimental model proposed, despite being an in vitro system, allows the derivation of useful information to predict the possible activity of the product in further in vivo application. Human fibroblasts (PEU cells) were treated with the product for 24 h at increasing concentrations of compared to control (untreated cells). The modulation of collagen synthesis was evaluated using a specific colorimetric kit (Sircol, Soluble Collagen Assay Kit). Increment of collagen production, 37.62% and 97.39% at concentrations of 1.25 mg/ml and 2.5 mg/ml of product, respectively, was considered to be statistically significant (*p values≤0.05 and **p values≤0.01) when compared with control (untreated cells). In conclusion, Hyaluronic Acid Hydrogel 26 mg/ml PEG cross-linked with calcium hydroxyapatite in low concentrations (1%) determines a statistical increment in neocollagenesis.

INTRODUCTION: Aplasia cutis congenita (ACC) is a rare congenital disease, characterised by absence of skin, that can affect different parts of the body. ACC is more frequent on the scalp, but can involve limbs, with partial-or total-thickness loss of skin, with a functional impairment of the joint affected. There is no clear indication for surgical or conservative treatment, therefore, it would be helpful to find scientific support for the optimal treatment strategies. CLINICAL PRESENTATION: We present a case of a newborn female, with six wounds on the inferior limbs, treated with conservative therapy. To prevent infection and promote healing, the defects were kept moist and covered with non-adherent/antimicrobical dressings. Complete wound healing occurred in eight weeks. The duration of follow-up was three years. RESULTS: In this kind of ACC with superficial partial-thickness wounds of the lower limbs, conservative treatment was successful. The same conservative treatment can be proposed for larger or deeper wounds of lower limbs with no involvement of underlying structures, with delayed scar excision after the complete growth of the patient. ACC scar excision could require complex tissue rearrangement, tissue expansion, or skin grafting. CONCLUSION: In the evaluation for treatment of ACC of inferior limbs, even in cases of total-thickness skin loss, even on joints, the conservative approach could be considered as the first choice. RESUMO

Post-burn contractures account for up to 50% of the workload of a plastic surgery team volunteering in developing nations. Best possible outcome most likely requires extensive surgery. However, extensive approaches such as microsurgery are generally discouraged in these settings. We report two successful cases of severe hand contractures reconstructed with free flaps on a surgical mission in Kenya. Microsurgery can be safely performed in the humanitarian setting by an integration of: personal skills; technical means; education of local personnel; follow-up services; and an effective network for communication.


Les rétractions après brûlures constituent 50% de l'occupation d'une équipe humanitaire de chirurgie plastique dans les pays en voie de développement. Les meilleurs résultats possibles nécessitent une chirurgie lourde. Cependant certaines de ces approches comme la microchirurgie ne sont généralement pas encouragées dans ce cadre. Nous rapportons deux cas successifs de rétractions majeures de la main, réparées par lambeaux libres dans une mission chirurgicale au Kenya. La microchirurgie peut donc être réalisée en toute sécurité dans le cadre humanitaire, à condition de bénéficier d'une expérience personnelle, de moyens techniques appropriés, de l'éducation du personnel local, des services de suite et enfin d'un réseau effectif de communication.

RESUMO

It is estimated that over 90% of people own a mobile phone. Although rare, lithium-ion battery explosions can cause varying degrees of thermal burn injury. Recently, selective enzymatic debridement has gained importance in the management of burn patients, thanks to its simplicity, minimum invasiveness and safety. In this work, we describe a case of a burn injury caused by the explosion of a lithium-ion battery and treated with selective enzymatic debridement in a paediatric patient.


On estime que plus de 90% de la population possède un téléphone portable. Bien que rare, l'explosion des batteries lithium/ions peut entraîner des brûlures de gravité variable. La détersion enzymatique a récemment pris plus d'importance dans le traitement des brûlures, grâce à sa simplicité, son invasibilité faible et sa sécurité. Nous exposons ici l'utilisation de cette technique chez un enfant victime d'un accident de batterie de téléphone portable.

A subject presenting a rare association of left labyrinthine and left hemispheric cerebellar lesion was examined, with recording and analysis of eye movements at different intervals. A complete compensation of the vestibular deficit and recovery of cerebellar postural ataxia, with persistence of limb incoordination and oculomotor abnormalities, were observed. The saccadic, smooth pursuit and optokinetic nystagmus systems were impaired. Ocular signs, in combination, may suggest a cerebellar dysfunction but they do not supply sufficient diagnostic criteria for localizing the cerebellar damage. Only the direction-specific impairment of smooth pursuit and optokinetic nystagmus is indicative of the side of the lesion.
